Third Party Insurance
Associations requiring additional 3rd Party Insurance for indoor/outdoor practice facilities or event, you must complete the 2025/2026 Third Party Insurance form below. Please make sure you follow all instructions referenced on the form:

Please note that 3rd party insurance certificates generally take two (2) weeks to process from the latter of the date of the receipt of application, payment and/or insurance affiliation.
Accident Reporting & Accident Claim Form
Please note injured parties have thirty (30) days from the date of the accident to initiate a claim with PWSA. The claim must be filed with the insurance provider before the 30-days expires. If an insurance claim is required please contact Cathy Bilinski direct at (905) 579-3002.

Note: Your team insurance certificate covers your team for accidents while on the diamonds only while playing in Canada – if you require travel accident insurance outside of Canada – you can use an insurance company of your own choice or contact info@pwsaontario.com for more details.
We continue to support through the P.W.S.A. insurance program:
- Affiliated Female teams
- Unaffiliated female teams that play house league, recreational, or competitive fastpitch/softball
- Coed teams that play softball (i.e. fastpitch/softball or 3 pitch)
We must advise you that under the terms of our Letter of Agreement between Softball Ontario and its Member Associations that we will be unable to provide insurance to:
- Male only teams playing fastpitch/softball, and
- Male or Female or Coed Slo-pitch teams
We encourage those Slo-pitch teams (males, females, coed) to consider the insurance program provided by Slo-pitch Ontario Insurance | Slopitch.org and all male softball teams to consider the OASA Insurance Insurance – OASA program.
